You are being asked to sign a work order electronically, on a phone or tablet carried by a driver or technician of the company providing your service (the “Company”). The Company uses software provided by Bond4Waste Inc. (“Bond4Waste”) to record the work and your signature. Your agreement for the service is with the Company, not with Bond4Waste.
Please read this disclosure before you check the consent box. It explains what your consent covers, what is recorded when you sign, and your rights under the federal Electronic Signatures in Global and National Commerce Act (the “ESIGN Act”, 15 U.S.C. § 7001 et seq.) and state laws based on the Uniform Electronic Transactions Act (“UETA”).
By checking the consent box and signing, you agree to sign the work order named in the consent electronically, and to receive that work order and the records related to it electronically. Related records include the record of the job's completion, the photos and notes taken for that job, and the Company's certificate of completion for it.
Your consent applies to that work order only. The Company may ask for your consent again for later work orders.
You are not required to sign electronically. If you would rather sign on paper, do not check the consent box, and tell the driver or contact the Company. Choosing paper does not cancel your service, although the Company may need extra time to complete or confirm the work on paper.
To show that the signature is yours and has not been changed, the following is recorded with it and kept by the Company:
A signature cannot be edited or replaced once it has been submitted.
You may ask the Company for a copy of the work order and the related records you signed electronically, on paper or as a PDF, at any time while the Company keeps them. There is no charge for a copy.
Before you sign, you can withdraw by not checking the consent box. After you sign, you can withdraw your consent to receive further records for that work order electronically by contacting the Company. There is no fee for withdrawing, and the Company will then provide those records on paper.
Withdrawing your consent does not affect the legal validity of a signature you have already given or of records you have already received electronically.
If the Company sends your records by email or text message, let the Company know whenever your email address or phone number changes.
You do not need any equipment to sign: you sign on the driver's or technician's device. To open, keep or print electronic copies of your records, you need:
If these requirements change in a way that could stop you from accessing or keeping your records, you will be told what changed and may withdraw your consent without charge.
